MMMF Application Process US/Canada

PLEASE NOTE: The application proess for the academic year 2012/2013 is now closed. The applications for the academic year 2013/2014 will re-open in Fall 2012.

 

A COMPLETE application package consists of the following (NO exceptions):

  • 1 (one) Online Application Form (see item A below). Click here to have a look at a draft application.
  • 2 (two) Online Recommendation Forms (see item B below)
  • 8 additional documents as listed in item C to be e-mailed.

    2. PLEASE NOTE: The application proess for the academic year 2012/2013 is now closed. The applications for the academic year 2013/2014 will re-open in Fall 2012.

  2. Online Recommendation Forms
    • Two (2) online recommendation forms are required. One must be by a professor from your present institution. The other can be by a former professor/supervisor from your country or elsewhere. One of the individuals must have known you for at least two semesters.

  3. All documents listed below have to be submitted
    1. One (1) passport-size photograph
    2. Picture page of your passport
    3. Copy of your US/Canada visa
    4. For applicants at US Universities, copy of I-20/DC-2019 (formally IAP-66)
    5. For applicants at Canadian Universities, copy of Study Permit
    6. Official estimate of annual expenses for students published by your institution
    7. Official copy of Fall 2011 or Spring 2012 Registration
    8. Official copy of transcript for Fall 2011 from your Registrar
